Kava is not federally scheduled as a controlled substance, and kava products are currently sold in the United States as dietary supplements and in some beverages. However, availability should not be confused with FDA approval. 

The FDA does not approve dietary supplements for safety or effectiveness before they are marketed, and manufacturers are responsible for ensuring that their products comply with applicable safety, manufacturing, and labeling requirements.

The regulatory category also depends on how a product is formulated, labeled, and marketed. A dietary supplement and a conventional canned beverage are regulated under different provisions of federal food law, even when they contain similar botanical ingredients. 

The FDA explains that a product represented as a conventional food cannot also qualify as a dietary supplement.

Can you build a tolerance to kava?

Human research has not clearly established whether regular kava use causes tolerance, meaning that progressively larger amounts are needed to produce the same effect. 

One older experiment reported behavioral tolerance in mice given repeated doses of kava resin, but animal findings cannot establish that the same pattern occurs in people. Researchers have studied how kavalactones move through the human body after single and repeated doses. 

A small pharmacokinetic study in healthy adults found that repeated dosing extended the duration of kavalactone exposure, but it was not designed to determine whether participants became less responsive to kava’s subjective effects.

Claims about “reverse tolerance,” in which kava supposedly becomes stronger after repeated use, are also based largely on consumer anecdotes rather than controlled clinical evidence. 

Changes in a person’s experience could instead reflect differences in serving size, kavalactone concentration, preparation method, food intake, other substances, or normal variation between occasions.

Because tolerance has not been adequately characterized in humans, increasing the amount simply to pursue a stronger effect is not evidence based. Consumers should follow the product’s serving directions and stop using it if it causes concerning or increasingly unpleasant effects.

Does kava interact with alcohol?

Kava and alcohol should not be combined. 

The National Center for Complementary and Integrative Health advises against using kava with substances that have sedative effects, including alcohol, because the combination may increase drowsiness, dizziness, slowed reactions, and impaired judgment.

Both substances also raise liver-related concerns. Alcohol is an established cause of liver damage, while kava products have been associated with rare but sometimes serious liver injury.

Is kava the same thing as kratom?

No. Kava and kratom come from different plants, contain different active compounds, and have substantially different pharmacological and safety profiles. 

Kava comes from Piper methysticum and contains compounds called kavalactones. Kratom comes from Mitragyna speciosa and contains alkaloids such as mitragynine and 7-hydroxymitragynine.

Kratom’s major alkaloids produce activity at opioid receptors, while kavalactones act through a different and more complex set of neurological targets. Describing the difference as simply “opioid receptors versus GABA” would be incomplete because kava appears to influence several signaling systems rather than one receptor alone.

The safety concerns are also different. The FDA warns that kratom use has been associated with substance use disorder, seizures, liver toxicity, and other serious adverse events. The agency also states that no FDA-approved prescription or over-the-counter drug products containing kratom are legally marketed in the United States.

Kava has its own risks, including sedation and rare reports of serious liver injury, but it should not be described as an opioid or treated as interchangeable with kratom. 

Because some beverages and botanical shots contain more than one active ingredient, consumers should read the complete ingredient list instead of assuming that every product sold at a kava bar or labeled as a botanical drink contains kava alone.

Will kava show up on a standard drug test?

Kavalactones are not among the substances targeted by standard federal workplace drug-testing panels. 

The Substance Abuse and Mental Health Services Administration’s federal workplace testing guidance describes testing for specified drug classes such as marijuana, cocaine, amphetamines, opioids, and phencyclidine rather than kava.

However, “standard drug test” does not describe every testing program. 

Private employers, athletic organizations, treatment programs, healthcare facilities, courts, and other institutions may use different panels or testing rules. A laboratory can also perform expanded or targeted toxicology testing when specifically requested.

Whether a substance appears on a test depends on which compounds or metabolites the laboratory is instructed and equipped to detect, not simply on whether the substance is federally controlled. For that reason, no one can guarantee the result of every possible toxicology test.

Kava products have also been linked to rare cases of serious liver injury, including cases involving traditionally prepared water-based beverages as well as concentrated extracts. The NCCIH advises consumers to be aware of this risk even though severe cases appear uncommon.

Possible signs of liver injury include unusual fatigue, persistent nausea or vomiting, loss of appetite, itching, pain in the upper-right abdomen, very dark urine, unusually light-colored stools, or yellowing of the skin or whites of the eyes.
